Mohsen Soliman Comments: All the comments are veryfor good, for excellent differential diagnosis. In the begining the case was suspected to be superficial fungus infection, that disseminated. However, negative mycology exam and nor response to systemic or local antifungal excluded the possibility. Vitiligo with an inflammatory wasanother possibility that was put forward, but was not confirmed neither clinicaly nor hisopathologicaly. Professor B.Ackerman in a visit diagnosed the case as subacute cutaneous type ofLupus erythematosus. So I like to congratulate the colleagues who clinshed the diagnosis, as she was the first case to be descibed in egypt, and this was some twent years ago. Thank you and we meet for the next case

Mohsen Soliman Comments: Thank you all for the comments, about this young 11 years old girl, who has got these annular lesion affecting the lateral aspect of the arm. The lesions were rounded well demarcated, scaaly at the marginand superficial. Mycological examination was negative, the lesions slowly progressed in couple of years and started to unite with each other, and in 4 years time it markedly increased in size. What would you suggest for the management. Histopathology examination demonstrated lichenoid infiltrate. Waiting for your comments after examination of the case after 2 & 4 years.

Mohsen Soliman Comments: Dear Dr Medhat Thank you for the comment, but the case was extragenital chancre that was followed by secondary rash, moist papules at the angles of mouth and conduloma lata around the scrotum and perianal area, plus psitive serology for syphilis. Dear friends thank you for the participaton, an as for a new case, it will be uploaded in few days, and was only delayed for little technical difficulties in uploading process. I am very much pleased that two of our colleagues reached the diagnosis of this difficult case.The case was Extra Genital Chancre secondary to the bite of His Syphilitic Girlfreind. The serological which were negative in the begining, changed to strongly positive. Also the patient developed clssic secondary syphilitic rash, moist papules at the angles of the mouth, and condyloma lata around the anus and the dorsal aspect of the scrotum. The patient received treatment and showed complete recovery. Nabil sidky Comments: what about repeating serological tests for syphilis. Extragenital chancre may be good suggestion. Please clarify the case more.
Dr. Altamimi Comments: The chancre is round or oval, approximately 1 to 2 cm in size, and has sharply defined, regular, raised, indurated borders. The “ham-colored” ulcer base usually has a smooth surface .may be transfer by infected mouth in this pt. After exclude the others. (my opinion)!
