Mohsen Soliman Comments:

Dear Dr Medhat Thank you for the comment, but the case was extragenital chancre that was followed by secondary rash, moist papules at the angles of mouth and conduloma lata around the scrotum and perianal area, plus psitive serology for syphilis. Dear friends thank you for the participaton, an as for a new case, it will be uploaded in few days, and was only delayed for little technical difficulties in uploading process. I hope you find it interesting, and I am thinking in making apresent for the best comment.

Dr.Medhat Raslan Comments:

I think this case may be a pyoderma gangrenosum or sq.cell carcinoma

Dr. Mohsen Soliman Comments:

Dear colleagues Thank you very much for the contribution and cooperation. I am very much pleased that two of our colleagues reached the diagnosis of this difficult case. The case was Extra Genital Chancre secondary to the bite of His Syphilitic Girlfreind. The serological which were negative in the begining, changed to strongly positive. Also the patient developed clssic secondary syphilitic rash, moist papules at the angles of the mouth, and condyloma lata around the anus and the dorsal aspect of the scrotum. The patient received treatment and showed complete recovery. The case will be left for a week, and untill we meet in the case of the coming month, Greetings from Cairo, and see you in our coming summer congress in Port Saeed, 19th- 20th July 2007. The details are available in the future events link.

Nabil sidky Comments:

what about repeating serological tests for syphilis. Extragenital chancre may be good suggestion. Please clarify the case more.

Dr. Altamimi Comments:

The chancre is round or oval, approximately 1 to 2 cm in size, and has sharply defined, regular, raised, indurated borders. The “ham-colored” ulcer base usually has a smooth surface .may be transfer by infected mouth in this pt. After exclude the others. (my opinion)!

Dr. Mohsen Soliman Comments:

Dr. Altamimi, please clarify your last comment.

Dr. Altamimi Comments:

ok . this chancre second my opinion

Dr. Mohsen Soliman Comments:

There was no history of specific illness that needed cryotherapy. Also the edges are raised and crusted, a character not seen with cryotherapy.

Dr. Altamimi Comments:

may be after crayotherapy by L.N

Dr. Mohsen Soliman Comments:

This ulcar was of 10 days duration serological tests for syphilis were negative at that time. Histopathology of the case was nonspecific

Nabil sidky Comments:

I think this case is lishmania (oriental sore ). What about tests for syphilis ?

Dr. Samar El-Tahlawi Comments:

I think the most propable diagnosis is Orf

Dr. Mohsen soliman Comments:

This ulcer on the dorsum of this adult 34 years old male, was of 10 days duration and was negative on mycological culture

Dr.Tim.W. Comments:

I think the case is Majochis granuloma. What are the mycological findings
